3 days ago I called up North Shore Hospital, radiology centre, to book in an appointment for my Mum. The receptionist texted me an email address to forward my Mum's imaging referral. In that email address I had to put in my name, mobile number and reason, which was helping my Mum request an imaging appointment asap.
Pretty sure my details were leaked through the contents of that email. North Shore Hospital needs to investigate this.

If on medication, bring a list of your current medication with name, dosage amount and number of times per day. IF YOU DO NOT BRING YOUR VALID CONCESSION CARD AND MEDICARE CARD YOUR APPOINTMENT WILL BE CANCELLED.
