    You have recently tested POSITIVE for COVID-19 and completed an online form. We need more information so we can determine how to best support you. Please complete this form via this link.

    Please complete this form as soon as you can. The information that you provide will be used to ensure that you receive appropriate care.     Fake text message asking for information about covid symptoms, please do not click on the link it’s a spam. Both myself and my husband received a message asking to fill out more information. NSW health will not ask for information via text message. - beware of the scammers.
